Job Summary

The Ambulatory Case Manager (ACM) supports patient care coordination in the primary care ambulatory clinic setting and/or in conjunction with the Managed Care Contracting identified goals. The ACM primary responsibility provides clinical evidence-based care management support to assist the delivery of effective education, self-management and timely health care delivery to high risk and rising risk patients associated with the assigned ambulatory clinic and/or managed care attributed patient population. The ACM works collaboratively with the multidisciplinary care team to deliver seamless transitions through the continuum of care by guiding the patient through the healthcare system in an efficient and fiscally responsible manner to achieve desired outcomes. The ACM facilitates alliance between the health care team, patients, family/caregivers and community by assessing, planning, and advocating for health care needs of an individual and/or identified population for quality, cost-effective outcomes. In collaboration with identified program and system leadership, the ACM will be responsible for identified data collection, management and analysis to assess the impact of the program. The registered nurse will demonstrate performance consistent with nursing and case management professional standards of practice, care, and the Nurse Practice Act.

Job Description

Essential Functions:

  • Enhance care coordination effectiveness through optimized communication and collaboration between hospitals, emergency departments, consulting physicians, and relevant community resources when necessary.
  • Partnering with doctors, providers, and healthcare workers to determine appropriate patients for care management. Using evidence-based resources related to care coordination and health transitions to aid in identifying suitable interventions for the enhancement and maintenance of health.
  • - Maintains a working relationship and awareness of community resources and payer benefits to link individuals with the most fitting resources.
  • Enhance coordination efforts by collaborating with the interprofessional health team in varied healthcare settings, ensuring financial accountability and actively advocating for the unique needs of each patient.
  • Partner with the patient and a team of healthcare professionals to assess the willingness for change, evaluate the patient's needs, devise a care plan, design educational programs, and track the patient's progress towards healthcare objectives and expected outcomes.
  • Recognizing and addressing barriers to learning and care delivery is essential for creating a patient-centered approach to healthcare. Making necessary adjustments to overcome these obstacles can improve patient understanding and engagement in their care. Providing guidance and support to patients and their families can facilitate the implementation of self-management tasks and enhance health outcomes.
  • Responsible for the shared development and monitoring of identified outcome criteria.
  • Undertakes current and retrospective clinical examinations to ascertain the best patient care strategies, emphasizing cost-effectiveness, appropriate patient care, and the efficiency of care coordination.
  • Forecasts the needs of the specified patient group, ensuring that necessary paperwork and pre-visit planning are finished or requested before the patient's visit. Efficiently uses available resources and takes into account cost effectiveness when helping patients schedule medical appointments.
  • Remains knowledgeable about the most recent developments, trends, and alterations in healthcare; routinely participates in continuing education and training activities within the realm of case management.
  • Extra duties may be required as directed by management.

Additional Requirements

Required:

Education - Completion of a nursing education program that is approved by a board of nursing

Certifications - Basic Life Support (BLS) Certification - American Heart Association (AHA) - Within 60 days of hire or transfer; Registered Nurse (RN) - South Dakota Board of Nursing

Regarding BLS certification: The department you are applying to may require BLS certification sooner than 60 days post transfer/hire. The hiring manager will discuss this with you if it is applicable.

Preferred:

Education - Bachelors degree in Nursing

Certification - Commission for Case Management Certification (CCMC) - Accredited University or accredited training professionals

Physical Requirements:

Performing tasks of medium effort entails the application of up to 50 pounds of force from time to time, or up to 30 pounds frequently, or up to 10 pounds consistently when moving items.
