Key Responsibilities for Care Coordinator (LPN):

• Manage admissions, discharges, and transfers of residents
• Coordinate medication orders with pharmacies and physicians
• Administer medications to residents
• Adhere to infection control, emergency, and facility policies
• Work with the interdisciplinary team to ensure the best possible resident outcomes
• Monitor and report any significant changes in residents’ health to the appropriate personnel as per facility protocol

Care Coordinator (LPN) Qualifications:

• Valid LPN license in the state
• CPR certification required
• At least two years of prior nursing experience preferred
